The case managers are assigned to specific medical-surgical units of the hospital and also cover the Intensive Care Units, Oncology Center and Emergency Department.
All inpatients are reviewed at the time of admission to ensure they meet inpatient admission criteria. Patients are continually reviewed throughout their hospitalization to ensure that he or she is discharged to the appropriate setting, at the appropriate time.
This position, by applying clinical expertise and knowledge of current utilization and discharge planning criteria, is responsible for ensuring that patients receive necessary services in the most appropriate care setting at CCAD.
Assesses patients’ needs relative to discharge planning and utilizes appropriate resources in a timely fashion
Serves as a liaison between the patient, physician and payer along the continuum of care
Determines the level of care appropriate for patient upon discharge
Ensures the completion of documentation related to discharge planning
Partners with physicians and nurses to ensure a comprehensive and coordinated plan of care
Works with physicians and nurses to correct utilization deficiencies and delays
Provides patient care information as requested to contracted insurers and/or reimbursement entities within the guidelines of the confidentiality information policy
Acts as an advocate on behalf of the patient to protect the patient’s health, safety and rights
. Determines the level of care upon request for admission utilizing evidence-based criteria
Bachelor’s degree in Nursing is required. * BLS is required *
Master’s degree in Nursing and Specialty certification preferred
Current national license is required
Minimum of two- (2) years of experience in case management * Minimum of five- (5) years of nursing clinical experience in an appropriate and relevant healthcare setting post qualification is required.
Acute care hospital experience and evidence of progressive leadership experience is preferred
Typical physical demands include: a high degree of manual dexterity to sufficiently perform computer functions on a keyboard, produce materials on a PC and operate basic office equipment, normal or corrected vision and hearing, mobility sufficient to visit with departments or employees throughout CCAD and lift up to 30 lbs.
